Your Health and Immunization Form, which displays your immunization records, is critical to your safety and well-being while you are enrolled at JCSU, particularly as we continue to live with the threat of COVID-19. Likewise, North Carolina law requires that all entering students submit proof of required immunizations to maintain enrollment at JCSU. The Director of Health and Wellness, Dr. Marian Jones, reserves the right to interpret the validity of all documents submitted to Health Services.
