The Veterans Coordinator manages the certification of VA Benefits.  Students applying for VA Benefits for the first time must complete VA Form 22-1990, Application for Educational Benefits.  If not on active duty the copy 4 of the DD Form 214, Certificate of Release or Discharge From Active Duty, must be submitted with VA Form 22-1990.  Students eligible under Chapter 1606, the New GI Bill (Selective Service), will submit DD Form 2384, Notice of Basic Eligibility (NOBE), issued by their Reserve Unit.  Students eligible under Chapter 35, Survivors’ and Dependents’ Educational Assistance Program, must complete VA Form 22-5490.  Veteran students transferring to JCSU will complete VA Form 22-1995, Request for Change of Program or Place of Training.  Transfer students receiving benefits under Chapter 35 will complete VA Form 22-5495.
