Student Pin
Any
currently enrolled student who needs his/her student pin or is
having trouble with their pin, should contact the Registrar’s
Office or the University Helpdesk (ext. 1398 or
helpdesk@jcsu.edu)
Transcript and Records
An
official transcript of a student’s academic records will be sent
to a designated person or institution upon receipt of a
Transcript Request Form
or written request by the
student. Telephone requests are NOT honored. All obligations to
the University must be cleared before a transcript is released.
Transcripts are
$5.00 per copy (money order or certified check only); $10 per copy
by telephone - payment by Credit or Debit Card.
All
transcript request are made through the Registrar's Office and
should be received at least one week before they are needed.
Processing time is 5 business days once the request has been
cleared through the Business Office. Request made at the
beginning or end of a semester may take longer.
The written request must include:

Name and any former name
-
Sender’s mailing address
-
Social Security or student id
number
-
Last date of attendance or
graduation date
-
$5.00 per transcript in the form of
a certified check or money order. Personal checks are not
accepted.

The signature of the person whose
transcript is being requested
and the date

Where the transcript is to be sent
including complete address
Permanent Records Folder includes the
original copies of all registration forms, drop/add forms,
incomplete removals, and copies of all grades students have
received at the end of each term.
Address and Name
Change
Address changes
can be done by completing an
Address Change Form
and returning it to the Registrar’s Office or in a letter,
including full name, social security number and student id number,
old address and new address. Any request for name changes must
include the appropriate supporting legal documents.
